Community Support

Our Community Outreach Team supports older people whose physical condition or functional mental health has deteriorated following a fall or bereavement or major life changing event resulting in isolation. We help them cope with their condition at home and reduce the risk of them being admitted, often unnecessarily, to hospital or prematurely entering residential care.
