Trusted Assessors

The Trusted Assessor project is funded by the Regional Partnership Board. It is a new project for Age Connects Morgannwg and focusses on supporting older people leaving hospital. The project supports the NHS Wales 6 Goals for Urgent and Emergency Care in Wales.


The main aims of the project are:

1: To help older people to get home from hospital safely and get well at home.

2: Find out how older people experience the hospital discharge process.

Our Trusted Assessors

We are your Trusted Assessors, based at Royal Glamorgan Hospital, Prince Charles Hospital and Princess of Wales Hospital.

We help by providing practical and emotional support to help you get home from hospital as quickly and safely as possible. We work with you, your family and in partnership with health, social care and third sector organisations to do this.


We are available on-site from  Monday-Friday between 8:30am-4pm.

We received a call from Mrs. F, a client seeking assistance with applying for a Blue Badge. Recognising the importance of this request, we promptly reached out to explain the application process and arranged a home visit to provide the necessary support.
