Our team works across all hospitals in Bridgend, Merthyr and Rhondda Cynon Taf, providing patients with the practical and emotional support needed to leave hospital safely, quickly, and with confidence.


We work alongside you, your family, and in partnership with health, social care, and community organisations to:



  • Support a safe discharge home
  • Reduce the risk of re-admission
  • Boost confidence and independence

Ongoing, free support to help people readjust to life at home

Practical help to regain independence and confidence

Tailored services based on individual needs

Reassurance and emotional support

Information and liaison with other agencies

Support for carers and families

Immediate food provisions until regular shopping can be arranged

Guidance with welfare benefit claims

Home safety checks on discharge – reducing risks of slips, trips and falls, clearing out-of-date food, ensuring heating, TV, phone and lifeline services are working

Our Trusted Assessors

We help by providing practical and emotional support to help you get home from hospital as quickly and safely as possible and preventing re admissions. We work with you, your family and in partnership with health, social care and third sector organisations to do this. Our support will enable you to increase your confidence and promote independence. 


We are available on-site from  Monday-Friday between 8:30am-4pm.
