Our Memory and Wellbeing Guide programme launched in 2024 with the aim of improving the lives of those diagnosed with dementia, as well as their carers. The scheme is part of the national Dementia Connector Programme and funded by Cwm Taf Morgannwg Regional Partnership Board. We are focused on meeting standard 12 of the All Wales Dementia Pathway Standards, and you can find out more about the Pathway by clicking here.
This project is dedicated to supporting individuals living with dementia and their carers by providing a named contact, known as a Memory and Wellbeing Guide. This guide will offer ongoing support, advice, and signposting throughout the entire journey, from diagnosis to end of life. We work closely with those with lived experience, to ensure that our programme is informed and shaped by those receiving the service.

We are currently in Phase One of the project, with Phase Two set to launch in 2025, expanding the scope and impact of our services.
What matters to you, matters to us. We understand that receiving a diagnosis of dementia can bring forth various feelings and many questions. We are here to help with navigating unfamiliar systems, by providing information, advice, support and referrals and a friendly, listening ear.
We cover the whole of Cwm Taff Morgannwg, including Bridgend, Rhondda Cynon Taff and Merthyr.
During phase one of the project, referrals are made to us through your health care professional, this may be from the memory assessment services or your GP. Once we have your referral, you can expect to hear from us within 48 hours.
