About Us

Age Connects Morgannwg is the leading charity working with older people living in Rhondda Cynon Taf, Bridgend and Merthyr Tydfil (the Cwm Taf Morgannwg Health Board footprint). We have a strong history with over 45 years of supporting older people and their families through some of the most difficult times in their life. We also have a proven track record of listening to older people, learning what’s important to them and caring about the way they are treated, perceived, and portrayed.

We are recruiting an experienced Programme Manager who will have significant knowledge and experience in managing and designing services for older people.


This new role offers the scope to deliver a newly initiated programme (Trusted Assessors) funded by Cwm Taf Morgannwg Regional Integration Fund, working closely with multi – disciplinary teams in acute hospital settings, to ensure that older people are supported to be discharged home safely and without necessary delays.


The programme will also include our Community Support Service which provides support to people at risk of premature admission to residential care or hospitalisation or those recently discharged from hospital who need a little help to get back on their feet. 



The role will manage a team of Trusted Assessors, Community Support Workers and Activity Coordinators and will be delivered throughout the Cwm Taf Morgannwg region which includes the local authority areas of Bridgend, Merthyr and Rhondda Cynon Taf.
