About Us

Age Connects Morgannwg is the leading charity working with older people living in Rhondda Cynon Taf, Bridgend and Merthyr Tydfil (the Cwm Taf Morgannwg Health Board footprint). We have a strong history with over 45 years of supporting older people and their families through some of the most difficult times in their life. We also have a proven track record of listening to older people, learning what’s important to them and caring about the way they are treated, perceived, and portrayed.   

About the Job

As a named contact for those in assessment process, newly diagnosed with dementia or those with an MCI, the Memory and Wellbeing Guide will ensure that:

 

  • People are connected to available and appropriate support networks, which may include peer support options.
  • People are able to access practical advice and information, including financial, to support with the adjustments to their lifestyle, or environment, that will help them remain as fit and healthy as possible.

 

The Memory and wellbeing guide as defined nationally will help people living with dementia coordinate care alongside the statutory duties of Health and Social Care.


RESPONSIBILITIES: 

 

The duties and responsibilities are informed by what matters to people living with dementia in the Cwm Morgannwg region. 

 

  • To establish the Memory and Wellbeing guide role as a point of access for information and advice to people living with dementia or MCI, throughout their journey and confirm the frequency of check in conversations they require. 
  • On first contact to facilitate a ‘what matters to me’ conversation to consider the person and their carer or family’s need for ongoing support and facilitate through the provision of information and advice and signpost as necessary. 
  • To be responsible for recording the ‘what matters to me’ conversation and the information and action plan agreed as a result.
