About Us

Age Connects Morgannwg is the leading charity working with older people living in Rhondda Cynon Taf, Bridgend and Merthyr Tydfil (the Cwm Taf Morgannwg Health Board footprint). We have a strong history with over 45 years of supporting older people and their families through some of the most difficult times in their life. We also have a proven track record of listening to older people, learning what’s important to them and caring about the way they are treated, perceived, and portrayed.

About the Job

Main Duties and Responsibilities:

Work under the direction of the Home First Programme Manager to provide practical and emotional support to older people to enable them to remain living independently in their own home for as long as possible.
Supporting older people in their own home, Community, and other settings, to regain and retain their independence and feel connected to their community.
To reassure, motivate and encourage older people to maintain quality of life.
To support the safe discharge of a service user to their homes from a hospital stay.
